It either came from Gaine or someone else that's still in the organization who wants to sabotage the new people in charge.
I wouldn't doubt Gaine was the source of Mando's and Jeff's info. He certainly knew what JI and Aponte and Joe were saying about each other. He certainly knew details about the GM search after JI was out the door. Then PFT put out a nugget that the report the Phins had completed interviews, when they hadn't, might have been a chance to smoke out the leak. I'm sure Ross didn't like the idea he couldn't remember an interviewee's name out there.
2socks commented that Gaine learned he was fired through the media, which you can see if you check back along the chain of posts I responded to.
One of the last executives Parcells brought with him from Dallas is gone. I'm surprised more people aren't celebrating.

When he was fired he was told to his face not through the media. Don't despair though.
Don't forget that by the third preseason game, the coaching staff knew that they didn't have anyone on the roster who could play any offensive line position except center in philbin's system effectively, as a result of which we could not run the ball or pass block, and nothing was done to correct it.
That's included in the second point. Sherman outsmarted himself a lot (as old OC's tend to do). He was calling games not based on his team or his opponent but things that caught him off guard o0r things he saw work in the past. I've been saying for awhile now that older OCs are generally bad for that very reason.
